Our contact form asks for your full name, email address, optional phone number, reason for contacting us, and message. When you submit it, the website sends those details by email to our office so our team can respond and help with your request.
Please use the form for general questions. Do not include medical records, detailed symptoms, Social Security numbers, payment-card information, or other sensitive information. The general contact form is not the patient portal. For questions about your care or sending medical information, call our office or use the patient communication tools provided by the practice. Do not use this website for emergencies.

Loading these resources sends technical request information, such as your IP address, to the servers delivering them. This does not mean your contact-form message is sent to those media services.
- Accessibility: The OneTap accessibility toolbar provides display and navigation preferences on the website.

Our website links to Elation Health for appointment requests and the patient portal, Zentake for online patient forms, Shopify for the online store, and Google Maps for directions. When you follow an external link, you leave this website. Information you provide there is handled under the notices and terms presented by that service, together with the practice’s applicable patient privacy notices. Review those notices before submitting information.
Third-party services can use their own cookies and collect information about activity on their services and, depending on their practices and your settings, on other websites. Your browser’s Do Not Track setting does not by itself stop the technical requests needed to display this site or control an external service. This website does not currently change its operation in response to Do Not Track browser signals.
